HELMETS ARE REQUIRED

Every trip participant must read and understand the cave's access requirements. The Trip Leaders must have each participant sign the cave access agreement hard copy form and have a copy of it with them on the premises during their visit. Each participant must willingly agree to comply to the requirements. Those groups that don't follow the requirements may lose future access and/or be cited for trespassing.

ADDING CAVERS & VEHICLES: If you decide to take other cavers or vehicles after submitting this form, then send the Cave Access Manager a private email with the changed information.

EMAIL ADDRESSES: Why do we ask for email addresses from the participants? We plan to start a newsletter of Nutty Putty Cave news and caving events and we want all of our participants to be included by receiving future issues.

PERMISSION SLIPS (MINORS ONLY): Participants under 18 must have parents or legal guardians read and sign a permission slip showing their consent and agreeing to pay for any damages and/or expenses related to rescue, extraction, and medical attention needed during the cave visit.

WAIVERS & MEDICAL RELEASE FORMS (ALL PARTICIPANTS): Every caving participant must complete the waiver and medical release forms. The waiver release liability and the medical release ensures prompt medical attention in an emergency.

WARNING: Caving is risky and all participants do so at their own risk and agree to release all liability to the cave land owners, The Cave Access Manager, and the Cave Management Team.

WAIVERS, MEDICAL RELEASES, PERMISSION SLIPS & BSA TOUR PERMITS

Trip leaders are responsible to have waivers, medical releases & any sponsoring organization permission slips secured before filling out his request form. The originals must be left with your emergency contact person at home, while copies must accompany the caving group in the cave. Failure to have these forms will result in great financial liabilities for the Trip Leaders, possible delayed medical attention for their participants, and the ultimate closure of the cave due to this blatant neglect.
