BLOWHOLE CAVE ACCESS REQUEST FORM
BlowholeNOTE: The same type of waivers, permission slips, and rules for Nutty Putty apply to the Blowhole Cave. Instead of an online reservation center, you can reserve your date and time by email.See the Blowhole Cave Trip Planner page for details. Due to the caving death and closure of Nutty Putty Cave, only Grotto led trips are being approved. If you are a non-grotto group, then you must contact the Timpanogos Grotto and get a qualified Trip Leader. Grotto led trip, require a 5 day lead time for approval. Follow the Blowhole Cave Trip Planner guidelines, and once you have the waiver, medical, and vehicle forms completed, then submit this Blowhole Cave Access Permit Request Form.
Helmets Required
HELMETS ARE REQUIRED

Every trip participant must read and understand the cave's access requirements. The Trip Leaders must have each participant sign the cave access agreement hard copy form and have a copy of it with them on the premises during their visit. Each participant must willingly agree to comply to the requirements. Those groups that don't follow the requirements may lose future access and/or be cited for trespassing.

EMAIL ADDRESSES: Why do we ask for email addresses from the participants? We plan to start a newsletter of Blowhole and Nutty Putty Cave news and caving events and we want all of our participants to be included by receiving future issues.

PERMISSION SLIPS (MINORS ONLY): Participants under 18 must have parents or legal guardians read and sign a permission slip showing their consent and agreeing to pay for any damages and/or expenses related to rescue, extraction, and medical attention needed during the cave visit.

WAIVERS & MEDICAL RELEASE FORMS (ALL PARTICIPANTS): Every caving participant must complete the waiver and medical release forms. The waiver release liability and the medical release ensures prompt medical attention in an emergency.

VERTICAL CAVE: The Blowhole Cave is a vertical cave that requires participants to know how to descend and ascend with mechanical devices. The BSA does not allow novice cavers to enter vertical caves. Special training and experience are needed before the BSA will authorize vertical caving trips.

WARNING: Caving is risky and all participants do so at their own risk and agree to release all liability to the cave land owners, The Cave Access Manager, and the Cave Management Team.

WAIVERS, MEDICAL RELEASES, PERMISSION SLIPS & BSA TOUR PERMITS

Trip leaders are responsible to have waivers, medical releases & any sponsoring organization permission slips secured before filling out his request form. The originals must be left with your emergency contact person at home, while copies must accompany the caving group in the cave. Failure to have these forms will result in great financial liabilities for the Trip Leaders, possible delayed medical attention for their participants, and the ultimate closure of the cave due to this blatant neglect.

By checking off the following approvals you are testifying that you indeed have these signed hard copy documents in your possession.

WAIVER/MEDICAL RELEASESS:
